WHAT IS THE 3SIXTY CARE PARTNERSHIP

The 3Sixty Care Partnership is a joint venture between a federation of 25 GP practices, a community and a mental health trust (Northamptonshire Healthcare NHS Foundation Trust - NHFT) in Kettering, Corby, Wellingborough and East Northamptonshire.

Working in partnership with NHFT to respond to new challenges in the delivery of modern primary care services. 

The partnership is uniquely placed to develop innovative employment models that support a wide range of workforce initiatives, bringing together health and care professionals to provide community based prevention programmes and joined-up health care and support for patients.

3Sixty Care Partnership brings together a GP federation made up of 28 GP practices across the north of Northamptonshire and the Northamptonshire Healthcare NHS Foundation Trust to form a multi-speciality Community Provider.
