PCNs are a key part of the NHS Long Term Plan

What are PCNs? 

Primary Care Networks (PCNs) are a key component of the NHS Long Term Plan, with general practices forming networks that typically cover 30,000–50,000 patients. These networks are small enough to provide the personal care valued by both patients and GPs, yet large enough to achieve impact and economies of scale through enhanced collaboration among practices and other local health and social care providers.

The 3Sixty Care Partnership currently supports five PCNs in the Northamptonshire area:

  • Red Kite PCN
  • Triangle PCN
  • East Northants PCN
  • Wellingborough & District PCN
  • Kettering & South West Rural PCN

PCNs provide the structure and funding necessary for developing services locally, tailored to the needs of the patients they serve.

The 3Sixty Care Partnership employs dedicated PCN Managers to support the Clinical Directors of the PCNs. These managers assist in delivering the contractual specifications, including the employment and management of roles available under the Additional Roles Reimbursement Scheme (ARRS).

  • Social Prescribing Link Workers
  • Pharmacists
  • Pharmacy Technicians
  • First Contact Physiotherapists
  • Nursing Associates
  • Paramedics
  • Care Coordinators
